for Swimming Lessons

IMPORTANT: Accounts must FREEZE from billing date to billing date. If you don't know the day of each month when your bill is processed, CCHC will send you your freeze effective start and stop date when freeze is completed.​

Please enter the start and end date you would prefer your account to be put on hold.

FREEZING POLICIES:

  • 1 week's notice required before billing date to make FREEZE adjustments

  • Fee to FREEZE is FREE if you have taken a lesson in the last 12 months

  • Must FREEZE from billing date to billing date

  • Must FREEZE all members or none

  • FREEZE up to 6 months in 1 year

  • You can freeze up to 6 months at a time

  • Membership must be in good financial standing

  • This form will be directly emailed to our guest services manager who will contact you with your next steps

  • You must contact our guest services manager to unfreeze or extend a freeze 30 days prior to any payment dates to ensure any payment changes

I authorize my regular membership dues to continue to be drafted in the stop freeze month (listed above) on the day when the dues are scheduled to be processed for my account.

I agree to all terms to FREEZE my membership. I also agree all information above is both accurate and binding.
